Dear 360ТА participant:

Thank you for participating in this 360ТА assessment feedback process. You will find the results rewarding, giving you a full and real perspective on how you and the others with whom you work perceive your effectiveness.

This 360ТА assessment tool consists of two forms: (Participant (Self) Form and Confidential (Other) Form. Each form contains two components: a numerical ranking and narrative comments on the same aspects of effectiveness.

Instructions:

 

1. As participant, you will complete the Participant (Self) Form.

    

2. Invite others (managers, those directly reporting to you, colleagues, customers, peers, board members, etc.) to fill out a similar form, the Confidential (Other) Form, which contains the same criteria as your Participant (Self) Form but is worded from their perspective. Print out a copy of the Favor Letter for them which explains the purpose of the survey.

    

Results from both the narrative and the quantitative data are compiled with confidentiality and anonymity. A full report lets you see how your perception corresponds with theirs.
